Training module 6:
Personal Planning in Social Services

This training is co-organised with Institute for Social Activities (ISA)

Purpose of this training

To strengthen the capacities of professionals working in the social protection service system and other stakeholders involved at the local level to develop personal plans in accordance with the needs of the service beneficiary/user.

Expected competences to be achieved

  • Understand the method from person centred care plan to personalized care plan
  • Understand and learn about service design principles
  • Understand the main steps to be taken in order to develop the personal plan
  • Strengthening capacities and skills of the care and support planning process and know roles and responsibilities of actors involved
  • Enhance capacities in monitoring the personal plans with the aim to learn planning is a continuous process and not the outcome.
  • Be qualified to write and work with a care plan

Trainer

Monika Gabanyi

Expert with more than 25 years of professional experience and engagement in managing projects, social and health system development, training and capacity building in the areas of long term care/ social inclusion/ integrated service provision in community care supporting people with mental and physical disabilities as well as the elderly.
